The Highlights:
Full-time, school-based Care Coordinator to the Mindful Middle Schoolers Program, a mental-health focused grant serving middle school-aged students and their family members in and around the Oak Park community. This role is responsible for coordinating and supporting behavioral health care, social emotional, and broadly defined wellness supports for students who are transitioning into middle school, currently enrolled in middle school, and transitioning to high school. This position focuses on engaging clients and their family members in their own care through consistent assessment, measurement of progress, outreach, education and referral management efforts. The Care Coordinator works across various systems to ensure that the needs of the whole child are supported as they transition in and out of middle school. The needs of the child and their family are at the center of the role.
What You'll Do:
Direct Services:
Conduct holistic needs assessments with each referred student and family;
Proactive linkage, referral and outreach to community providers, school systems, and supports using systems such as the Health Connection Hub;
Support and closely coordinate behavioral health care needs (school-based providers, PCP and, when appropriate, other community-based mental health providers);
Home-visits as needed to support children and families.
Progress Monitoring/Data Collection:
Progress monitor clients (in person or by telephone) assessing need for continued care, new supports, and efficacy of current supports (data tracking);
Gather input from students and family members to inform programming and services.
Documentation and Administrative Support:
Coordinate intake and documentation process for school-based programming run by DePaul Family and Community Services.
Maintain timely and complete documentation of services and program outcomes;
Provide other administrative support for programming as needed
Meetings:
Prepare for and participate in regularly scheduled consultations with members of the local school district social emotional support teams and other community providers;
Participate in regular supervision meetings
What You'll Need:
Bachelor's Degree in Social Work, Psychology or related health field.
2 years of experience with children, adolescents and families, especially urban, under-resourced, and culturally diverse populations.
Capacity and interest in working within school-based, community-oriented team model.
Experience working with issues such as trauma, mental health, and substance abuse.
Working knowledge of evidence-based psycho-social treatments and case-management practices.
An understanding of systems, communities, and the importance of collaboration and integration.
Must be mobile enough to travel to community sites (car is required, mileage reimbursement provided)
Basic familiarity with computers.
Use of personal vehicle. Some late evening and weekend work may required in order to meet the needs of clients and their family members.
Note that this position is funded by an external grant source. If the grant terminates for any reason, if grant funding is lost or reduced for any reason, or if the budgetary needs for fulfilling the grant requirements change, your employment with the University will be immediately discontinued.
